Incident and Reporting Policy

General Guidelines

All employees of the FAU Libraries have the following guidelines for action in any emergency:

  • KEEP CALM.
  • Quickly gather as much information about an incident as soon as possible and within a reasonably short period.  This information can include but is not limited to: the nature or description of event; patrons or witnesses to the incident; its date, time, and location; and other relevant details.  This information will be included on the Incident Form.
  • Evacuate the area if the threat of danger is imminent and secure the area from entry.
  • Call the appropriate emergency agency (police, fire, or ambulance) by calling 911 and stand available to direct them to the source of the problem.
  • As soon as possible after the incident, notify your supervisor and complete the Incident Form.

All library employees are required to report all accidents or unusual incidents.  Include any information on the Incident Form. The report should also list the names of any library employees or patrons who may have witnessed the accident or incident; if names are not available, descriptions can be provided. 

An incident may include but is not limited to: harassment, problem patrons, or inappropriate conduct.

An accident may include but is not limited to medical emergencies such as serious falls, cuts, unconsciousness, or seizures. 

If an incident or accident involving a patron or staff occurs on library property or off-site during library programs or services, or if there is doubt about the safety of those present during an incident or accident, 911 should be called.
