The FAU Libraries Promotion Guidelines Submission Form is an online form used by library faculty or staff to submit promotion-related guidelines and supporting documentation for review and evaluation. Safety Training for FAU Libraries staff (Only).

Users can recommend books, journals, or electronic resources for addition to the Medical and Health Sciences collections. The form collects details about the requested item and its relevance to academic or research needs. Submissions are reviewed by library staff to support collection development and ensure resources align with the instructional and research priorities of the university.

Please use this form when a patron situation rises to the level of an “incident.” An incident could be one of the following :

  • A patron’s health or safety is threatened.
  • Harassment of a patron or a staff member.
  • Emergency services or Police are called to the building.
  • An urgent situation which requires that Library administration be notified.

Staff can use this form to checkout IT equipment 

FAU-Boca Raton (FAU-BR) faculty members can request a literature search for a research project

*Please note that a minimum of 5 business days notice is required when requesting property removal. The item(s) you are requesting to be removed must remain in the location you enter on the form. Hector Moyet will contact you when he is notified of the pick up date for the item(s) by FAU Property.
