This form is for all FAU faculty using e-reserve. Please complete this form in its entirety and submit one form per request. If electronic permission is denied by the Copyright Clearance Center (CCC) then the Reserve Team will make every attempt to secure permission to use paper. Please allow up to 7 days for reserve requests to be processed.

The Digital Library may provide digitization services to FAU faculty, staff, and students, as well as external researchers upon request. Use this form to submit requests.

Special Collections Instruction Request Form

We're glad you're interested in having a presentation or a class using Special Collections materials. We do ask for a request form and two weeks notice so we can prepare for your class and make it the best session possible.
